Rentosertib, an experimental drug created by biotech company Insilico Medicine with the help of artificial intelligence, was designed for an incurable lung disease. During testing, researchers found something they were not looking for: the indicators used to estimate patients' biological age changed.

The drug targets idiopathic pulmonary fibrosis, an incurable condition in which scar tissue forms in the lungs and breathing gradually becomes harder.

## How the molecule was found

AI first helped identify a protein that might be involved in the disease and could be blocked with a drug. The system pointed to TNIK, linked both to the development of fibrosis and to some aging processes. Another system from the company, Chemistry42, then helped design a molecule capable of blocking TNIK. Further selection and testing produced Rentosertib.

## What the aging clocks showed

After the tests, the researchers decided to check separately whether the drug affected signs of aging. They analyzed patients' blood samples and estimated biological age from changes in the levels of various proteins, using several so-called aging clocks. ProtAge, a machine learning model, estimates a person's age from a set of blood proteins, while OrganAge includes models that use the same data to estimate age or the associated risk of death.

In patients receiving Rentosertib, these scores shifted on average toward a younger biological age. In the placebo group there was little to no such effect. By one method, the difference came to about six years.
